Monthly Point Forms
Point forms must be filled out by the tenth of every month
by either the Club Secretary, President, or Advisor.
The form must be sent to your Lieutenant Governor, District
Secretary, District Governor, and District Administrator.
It is also a good idea to keep one copy for yourself.
If you submit the form online it will be automatically sent
everywhere it needs to go.
